Sign ups are now open for 4-6th grade Basketball for the 2024-2025 season!
Registration closes on Friday, November 1st*.
The fee to register is $50 per child or $75 per family.
*Accepted forms of payment are check/ money order made payable to Demarest HSA or online through our the Demarest HSA Store.
Team uniforms will be provided. You will be required to sign a waiver which states your child will not wear their uniform to school or at any time other than the designated games/tournaments. You will also be responsible for the full replacement cost if the uniform is not returned at the end of the season. In order to participate, you will need to provide us with the following six (6) items:
1. Registration Form (current page)
2. Completed Permission & Insurance Form (page 2)
3. Basketball Uniform Waiver (page 3)
4. Photo Release (page 4)
5. Copy of Insurance Card: Please email a photo copy of your child’s insurance card (front & back) to basketball4-6@demaresthsa.org once this application has been filled out and submitted.
6. $50/child or $75/family: Check or money order made payable to Demarest HSA ($25 returned check fee applies). Online payments now available! Please send in the check/money order in an envelope labeled 4th-6th Grade Basketball with your child’s name and their teachers name, and return it to school.
**IMPORTANT: Children will not be able to participate without insurance
In order to participate, please complete the below registration form, email a copy of the front/back of your child’s insurance card and and submit payment to secure your child’s spot for the 2024-2025 season.
