Registration is now open for 2nd & 3rd Grade Soccer Clinic! Clinic will be held on MONDAYS AND WEDNESDAYS from April 29 – May 22 in the back of the Demarest School. Deadline for registration is Friday, April 19. For more information or to volunteer, please email: soccer@demaresthsa.org.

In order to participate in the clinic, please complete the following :
1. Completed Registration Form
2. Completed Code of Conduct
3. Completed Permission and Insurance Form
4. Completed Photo Release
5.Copy of Insurance Card (Front & Back) – email to soccer@demaresthsa.org – you will receive a confirmation email. Please note: Children will not be able to participate without insurance
6. $25 – Check or Cheddar Up (online payment) – Check made payable to Demarest HSA – in an envelope labeled “Soccer Clinic”, with your child’s name and their teacher. This can be sent into school with your child. ($25 returned check fee)
